The face and upper neck are the most commonly affected, but the chest, back and shoulders may have acne as well. The upper arms can also have acne, but lesions found there are often keratosis pilaris, not acne. The typical acne lesions are comedones and inflammatory papules, pustules, and nodules. Some of the large nodules were previously called "cysts" and the term nodulocystic has been used to describe severe cases of inflammatory acne. True cysts are rarely found in acne, and the term should be abandoned and the term severe nodular acne used instead.
Benzoyl peroxide primarily works as an oxidizing agent, which allows oxygen to penetrate through the pores and into the skin, creating an oxygen-rich (aerobic) environment. Hence, Benzoyl peroxide is an antiseptic as it indirectly kills Propionibacterium acnes - the culprit for acne outbreaks. As this medication reduces the quantity of bacteria on the surface of skin, it does not however trigger bacterial resistance or adverse reaction to antibiotics. Moreover, Benzoyl peroxide also deters the multiplication of yeasts on the skin.
As mentioned earlier, although not perhaps the root of acne, bacteria and clogged pores are a primary causes for concern. The processes discussed here ensure we are not promoting an environment in which bacteria can flourish.
